Passport Bio-Page
Submit a clear, high-resolution scan or photograph of the bio-page of your passport. This page should clearly display your full name, passport number, date of birth, nationality, and passport expiration date with at least 06 months validity .
Passport-Sized Photograph
The photo should be taken within the last 6 months to reflect your current appearance with clearly visible.
e-Visa Application
Download the form thru link provided here. Complete the form accurately and submit it along with hospital Visa invitation letter document. For any assistance connect with us.

Please ensure that all documents are legible and complete. Incomplete or unclear submissions may result in delays. Your initial Visa will be valid according to your treatmeant plan as per doctor recommendation. If your treament plan is extended beyond your visa validity then do not feel worry we will be happy to assist with the extension process.
